of Star Trek-not including the civil war episode(I have no idea the death toll on that one). I'm pretty sure that was on Voyager.

The was a Q that was allowed to leave the Continuum and he later committed suicide. I do not recall seeing any other Q's die beyond that Civil War episode.

The was an episode of Star Trek: The Next Generation called True Q where a storyline included a referenced to a couple of cue that had been killed years before. 2 Q wanted to become human so they "renounced" their powers to live on earth...the couldn't resist the temptation to use their powers and they were killed by the Continuum. They had a daughter who appeared to be a normal human child, but she developed Q powers.
