Nearsightedness which is known as Myopia is a condition in the eye where long distance vision is out of focus due to the lens in the eye and sometimes the shape of the cornea and eyeball focusing objects in front of the retina instead of on the retina.
Farsightedness which is known as Hypermatropia is a condition in the eye where long and short distance vision is out of focus
due to the lens in the eye and sometimes the shape of the cornea and eyeball focusing objects past the retina instead of on the retina.
To correct Myopia a concave shaped lens (thicker at the edge and thinner in the center ) will focus the image on the retina.
To correct Hypermatropia a convex shaped lens(thicker in the center and thinner at the edges) will focus the image on the retina .

The condition is usually caused by an elongation of the eyeball that occurs over time. Farsightedness (hyperopia) is the opposite of myopia, and is usually caused by shortening of the eyeball.
Astigmatism is an imperfection of the cornea preventing part of it from focusing light onto the retina. The result is a blurred area within an otherwise clear image. This problem may occur along with either myopia or hyperopia. These frequent conditions are termed "refractive errors."

Nearsighted is when everything up close is clear, but everything further away is blurry.
Farsighted is the opposite. Everything up close is blurry, but everything far away is blurry.
Farsighted people usually only wear glasses for reading, whereas Nearsighted people (depending on how bad) need to wear glasses for everyday use.
