Seems like sometimes an explanation winds up making you look guilty, and you're trying to come up with an excuse.

I think an explanation is always is always a good idea, because you do it with good intent and in order to show the other person how much you care about them and what happened was only a misunderstanding....however, unfortunately, a lot of the time it seems that explanations only make things worse, because the other person cant figure out why you´re trying so hard to explain something to them and so they think you´re just trying to trying to clear your own conscious....communication can be messy like that sometimes....

It depends on the situation. Sometimes things require an explanation (e.g. why you would not remove Phigment from your contacts) while some do not (why men worship you for your beauty).
