About as often as evil does.
Luckily, in general the memory of good things prevail over the memory of less desireable ones.

Well, that's a really interesting question you propose. To be truthful, I think the foundation of the question is a bit flawed. You are concerned with an objective good ultimately beating out that which is evil. Whether or not this is the case is completely indiscernable because we don't have a wide enough perspective on things to see if some universal good triumphs over evil.
What we can say, though, is that which prevails is likely to be believed as good. "History is written by the winners" is a phrase that comes to mind that seems particularly relevant. Meditation on justice, which is what you are doing, would be devoid of meaning if you did not consider this point.

Of course not. The bad guy has gotten away many times. Some injustices have been, prov en recently with DNA. Showing how easy it is to die in prison an innocent man. How many have? How many will? Good didn't prevail for them. Presently good is not prevailing in most of this world. Nine US soldiers killed today in Irac, so commen you probably didn't hear. It's survival of the fittest, sometimes the bad is just too strong.

Always and never.
Reality of who "prevailed" is irrelevant. Your attitudes and perceptions are what, in the end, matter.
If you subscribe to the notion of an objective reality and your attitude is that good does prevail, then it will consistently do so.

For good to prevail, it must be supported by something else.

Has it in the past?
...not really
But if you believe in the Bible (Revelations), then you would believe that good will ultimatley triumph in the end.
By the way, what is good? To different people it means different things...
