Most likely your teacher sees this as a form of therapy for you. You don't have to share anything that is not relatively public information. Use these papers to try to learn something about yourself or to grow in some way. I always found these types of papers to be very cathargic.
In my exprience, most instructors can be trusted, but you don't feel comfortable with this instructor change names or details slightly "to protect the innocent (or guilty)".

You do not need ot reveal things that are not 'publically' known. Your bra size is a secret! However, what is required is the cute personal details like how your dad used to tickle your ear as he said goodnight and interesting details and stuff like that.
Hey! Make it up if you like. It is the writing that is the important part, not the facts themselves.
Use colorful adjectives, and never say "nice". Ever! It should be struck from the dictionaries.! Make the verbs zing!
