Same question came up a day or two ago. For MLB, the answer is Tom Cheney, who struck out 21 in a 16-inning complete game in 1962 (box score: http://www.retrosheet.org/boxesetc/1962/B09120BAL1962.htm).
For a standard nine-inning game, Clemens (twice), Wood, and Johnson (pitched the first nine of an 11-inning game) had 20.
The minor league record is by Ron Necciai (pronounced "net-shy"), who rang up 27 Ks in one game in 1952.

Tom Cheney had 21K's in a 16 inning game pitching for the old Washington Senators in 1962. Kerry Wood and Roger Clemens(twice) had 20 in a 9-inning game. Randy Johnson also struck out 20, but in 11 innings.
