It's Washington, DC.
The other cities mentioned, like San Juan, in Puerto Rico, are not US cities, since they are not in the United States of America, the nation.
They are territories of the USA, which simply is the same as a state or the District of Columbia.
No one in Puerto Rico is going to say they are American, although they are American citizens. Also, no one is going to consider them Americans on their being in Puerto Rico.
So, it's just DC.

The answer you're looking for is probably Washington, D.C. (as it's the only one in the mainland that's not in a state).
However, cities from U.S. territories (which are not states) are also U.S. cities in a sense. So, San Juan, PR, Hagatna, Guam are both cities not in a US state also.

Be warned however that not all the 'states' are um...states...four of them in fact...
So, the next time someone asks you how many states there are feel free to say fourty-six...but quickly add, and four commonwealths (KY, MA, PA, VA)
