Is it only when penetration occurs, is it holding hands, kissing, french kissing,cyber sex, whispering sweet nothings in a dance partner's ear, taking a "friend" to lunch? Or is it a state of mind? I hate that one because, what if I were thinking of you the whole time? Where's the line?

This is a tough one, especially for those of us with a firm moral compass.
I don't think a "state of mind" can be considered cheating. Even Jimmy Carter admitted to having "lusted in my heart". We are all human, after all. And fantasies never hurt anyone; they can even be seen as an aspect of a normal sexual outlet among long-term partners.
As for the other parameters listed, this is an individual issue, to be dealt with by the specific persons involved. Expectations, desires, belief systems all play a role in determining appropriate behaviors.

Does this question perhaps belong in "Philosophy" or "Marriage and Relationships"?
