An extra inch around the bust. So both 34D and 34DD will have the same ribcage measurement, but the 34DD will have a 39 inch bust and the 34D will have a 38 inch bust.

The extra D means that the girl has a larger cup/breast size then the regular 34D. So, she needs more room in the cup of her bra.

It just means she needs extra room and her bust is more fuller. Like myself im usually a 36 C but since im a bit more fuller i have to go up a 38C.
