I can tell you when they should be turned on, only when you're able to answer them, common sense should be a factor, but for some reason people think it doesn't apply when they get a phone call.

You don't have to turn them off, but you should put them in the silent mode, (vibration), while in important business meeting, church, movies, and while doing anything else, where a load phone would tend to alarm or decrease someone else's, enjoyment or fullfilment of the moment, like watching a movie, play, graduation exercise, school, wedding and funeral. Some people would think it would also be improper to have in own while visiting a hospital, nursing home, eating a fine meal or just eating dinner with your family. I am sure there are others times, but considerating others is the reason it should be silent.

Any public area that demands quiet, such as libraries, movie theaters, performances (not loud rock clubs, though!) It is also rude to talk on the phone when you are paying a cashier for your purchase. It is rude to have your cell on when you are having a heavy conversation with someone or during sex. Also, if you have your phone on at a restaurant, turn the ringer down so you don't blast out other people when it rings.

Whenever you are in a courtroom. Usually there are signs outside the courtroom and judges chambers about shutting off phones. I have seen judges so mad at ringing cell phones that the bailiff takes the offending cell phone away from the person until the court proceedings are finished --in a large traffic court that can be hours.

Turn off cell phones- in a hospital or doctors office, church, at a play or movie, in school, at meetings, job interviews, at work for some people, I'm sure there are others I haven't thought of.

At work while working. In a dining room or restaurant. In a theater, movie or otherwise. In a presentation. during a graduation. during a recital, during anything that you should be paying attention to...when your phone rings in the middle of my presentation, that means that you don't care to listen to my message and that you're not interested in the material.
Cell phone rings are a disturbance in restaurants (not McDonald's, but real restaurants) and in movie theaters...if it rings, don't answer it, just shut it off...nothing worse than having a yacky you know what behind you while you paid $$ for the joy of watching a movie in a theater...

i work in a restaurant and i think it is so rude when I am trying to take an order from someone and they are on the phone, also i see so many parents take their kids to lunch then ignore them and talk the whole time, or even with a partner. No one wants to go to lunch or dinner with someone that is going to be on the phone with someone else the whole time. that's very rude.
the rest of the places have been covered by everyone else.
