Eraserhead - hands down. I've seen all the rest and this one has them all beat as far as weird and f*'d up goes.
My runner up to that would be Basket Case. The very last scene in that movie is about as messed up as you can get. It's an old B movie - but ewwwwwwwwwwwww.
Third place would be Hostel.
