Aware of the state and condition of self to a fault, or to their detriment or to the detriment of being able to see anyone else's state or condition past their own...
But what it means literally is a person who is so concerned with the self that they are paranoid or insecure as a result.

closed off, constantly worrying about how you look, how you are perceived, how you sound, how you come across to others. irony here is that you end up looking worse than if you hadn't worried so much

Simple, closed off.
They need to open themselves up to others as well as to them selves.
