How about a gift certificate to a classy day spa? Massage, manicure, make over are all services they should offer.
Do your homework and find a nice one with ambiance; there are too many shlocky ones out there.
She'll love it!

A charm bracelet with a "21" on it and also a "friendship" charm.
Or perhaps a photoframe/album with pics of her childhood or the 2 of you in it?

go to tagret and get her a girly martini set (pink) they have lots of cute varieties. fill the cups with candy, scratch tickets, jewlery, little mini liquor bottles, lip glosses. little fun things to make someone feel special and luck to have their b-day!
