Sometimes, when you hit the low points in life like she has, you become overly upbeat to keep from killing yourself. She is probably on auto pilot.
Nice of you to want to help.
Contact your church. If you don't have one, start contacting a lot of churches in your area and tell them the situation. They may be able to help. Or at least point you in the right direction. ALSO...contact the family hotline in your city. They frequently have emergency housing to help out people like this. If she's got four kids already, it's probably tough for her to make the time to make the calls. Do the calling for her.
Good luck and hope this helps!!

I wouldn't even get involved. People like that are usually in bad situations because they make a lot of bed decisions. You might offer her a place to stay and that will end up in 3-6 months of hell. Where is the father of her children? That's a huge question among others. Give advice only, but keep it on a casual level.

Take her to planned parenthood and get her on the pill!!! If she can't afford the ones she's got, then she shouldn't be having anymore. Sorry this is so blunt but I would have loved to have lots of kids but in all reality, I knew I couldn't afford it. I don't live off welfare or any other kind of government but we still struggle to make ends meet. I wish her luck but 5 kids is just a little extreme don't ya think?

wow, that is sad! It makes you put things in perspective! Maybe try to raise some money and food for her from neighbors, friends and family or have a fundraiser for her like with a dinner or something. it is nice that there are people like you who care

Wow, I think right now she really needs a friend. I don't know if you can offer your home. But we all can use a cook, maid....or something maybe to earn her keep. But most of all she needs an ear and/or a shoulder.
