early 1977, Frank Price, head of Universal Television, offered producer and writer Kenneth Johnson a deal to develop a TV show based on any character in the Marvel Comics library. Johnson turned down the offer at first, but then, while reading the Victor Hugo novel, Les Miserables, he became inspired and began working to develop the Hulk comic into a TV show. Johnson first changed the name of Dr. Bruce Banner to Dr. David Banner. This change was made, according to Johnston, because he did not want the series to be perceived as a comic book series, so he wanted to change what he felt was a staple of comic books, and Stan Lee's comics in particular, that major characters frequently

Bruce Banner in the comics is actually Robert Bruce Banner, after the ancient Scottish hero Robert ‘the’ Bruce. However, Bruce’s father who abused him and his mother was also Robert, so it is no wonder he went by his middle name. The name David came from the TV show. They often make alterations in the characters, so that both the TV and the comic book firm have separate marketable properties for spinoff and merchandising purposes. Sorry, hope this isn’t too blunt and illusion-wrecking.

The wiki link has the best answer but i just had to add my two cents anyway...
Since the TV show the Comic book version Hulk (And Banner) has used David as an alias. And The Hulk's full name is "Robert Bruce Banner".
The more you know!

Back in the late 70s, early 80s, "Bruce" was considered the epitome of a homosexual name. So -- "Bruce" was too gay. I am not kidding.
In the comic books, Hulk's name is "Robert Bruce Banner" due to an early-on accident in which the writer, Stan Lee, forgot his own character's name (Bruce) and was referring to him as "Bob Banner." (Pretty funny, Lee had the same mistake with Spider-man, and called him "Peter Palmer" but that time the new name did not stick).
