ask the parent if theres anything they dont have that they need like a stroller or maybe one of those rockers or mobile baby sets. Also baby swings for the swing set are ahdorable. Cute clothes for the boy is also good. A little play set like a plastic one is cute like with a little slide that might be a little too old for him tho

Ask the parents if there is anything in particular they want or need for the baby.
To give you some ideas, this is what my son recieved for his 1st birthday (which was 2 months ago) :-
- Thomas the Tank Engine Flip out Sofa
- Clothes
- A Wiggles Backpack
- Books
- A Fisher Price Little People Pull-Along Musical Zoo Train
- Winnie the Poo cot sheet set
- Thomas the Tank Engine & Wiggles DVD's
- A Ride on Bike

- A pair of shoes

How about a savings bond? I buy one for every birthday & christmas, and I figure that it will be one heck of a downpayment for a good car by the time they're ready for college, or maybe even a house. You need his social security # to buy one (you can get them at any bank). I buy $50 ones, (you buy them for half of the face value, so a $50 bond costs $25) , and they mature in 7 years, and then begin accruing interest beyond that 7 yrs. Other than that, I would recommend getting him a big empty cardboard box, and a ball. At 1 yr, the simple things are still the best.

Books!!! Books!!! Books!!! Board books that is.
Also, if you are looking for a toy of sorts - try a bubble lawn mower - since he'll be toddling around this summer.
Or sand toys for the sandbox - and if he doesn't have a sandbox, that's an option too. They make the ones in the shape of a turtle and such.
Or a pool with pool toys and a bathing suit.
Or a bunch of cars/dump trucks/construction vehicles.
Or DVD's that are age appropriate.
Or a wagon or outside tyke car that mom/dad can push with a handle.
****Lots of ideas will be thrown at ya I'm sure but just keep it really simple. Don't get something with lots of pieces. Make sure it is something that will challenge his mind. They need all the stimulation that they can get at this age!****

I would go with what most of the people on here say see if there is anything that he needs or if not then if the mom and dad are OK with it then find him some toys that make different sounds and also talk I'm sure he'll love that because most baby's at that age love things that make different sounds .
Oh and if the mom and dad did his hand print when he was a new born they have one for when they turn a year old that's what my sister got for my daughter she found it at one of those party stores.

Something that they're going to end up buying him anyway. Maybe a new carseat, a walker, clothes, shoes?
Like I said something practical. But you mentioned something special, let's see...
Wow, I don't know. How close are you?
If it's your nephew or something - How about you write him a story? Something you or his parents can read to him. When he gets older you guys will have a great memory to share.
If you're not that close and that seems cheesy maybe you can make "mosaic" of pictures of his first year. Kinda like a scrapbook page but bigger, with more pictures, and frame it. It will mean a lot to his parents.

Most parents will prefer you buy something the child needs. Probably clothes, diapers, or sippy cups. That's what I'm asking for, for my daughter's birthday.

if they have a yard
my boys always loved those little plastic lawn mowers. Some blow bubbles which is extra cool. or a kids wheel barrel and gardening tools. Im big into gardening so i always get my kids started early. If he can walk he can push a mower.
or
my kids also really loved those fisher price little people.
