Wolverine has sever anger issues that get the better of him..alot but he still has a strong sense of justice. He definitely teeters on the edge of sanity and oblivion.

The Punisher is probably the best example. While he's not entirely insane, he has a very thin view of morality and is quick to kill or maim anyone he considers a criminal. So while he is a regular murderer, he is also bent on destroying anyone who breaks the law or murders others.
Definitely a classic psycho hero.

The Badger. He was literally insane (see http://en.wikipedia.org/wiki/Badger_%28comics%29 for details)
Moon Knight and Azrael are both insane as well.

Not exactly a SUPER hero, but more like the protagonist of the story (that means good guy right?) My example is Naruto from.. Naruto lol. He's stupid, he's dim-witted, he loves to eat sleep and beat things to a pulp. Yet he has rescued tons of people from death and more from suffering. He's failed his ninja class a bunch of times and has a need for acknowledgement, he has a big disadvantage because of that. Yet many praise him because of his hard work and determination to save all those who have been trapped, from the darkness they've been exposed to.

Batman for one, not only is he obssessed with not letting the criminals get away because of his parent's demise at the hands of one, but who else do you know runs around wearing his underwear over his pants and puts a belt on it to boot.
