Many PhD programs in epi won't take fresh masters students. They'd prefer if you worked in epi first before applying. So if you want to work in thalassemia, I'd recommend you find a hematology, cancer or genetics research org (lab, hospital or otherwise) to work for. Extra points if you can find work in a university with both a PhD program AND a suitable research group.

Demonstrate that you can do the real world work. If you work at a university with a desirable PhD program, get to know the Epi faculty. Yes, personal acquaintances can help you get in faster. Try to get a position in their research group.

Once you have two years under your belt, then apply to the PhD programs that look good to you and I'd bet you'd be a shoo-in.

You sound smart enough to know already.....But seriously, discuss with a counselor at school or a professor could direct you.
