Cookies are information stored on the hard drive by a web site. Examples of stored information include your preferences for a particular website etc.
For example you visit your Google or Yahoo homepage and it remembers you, and shows content that you selected it to show you. This information is stored on your hard drive in shape of cookies.
Now, to answer your questions one by one:
a. Yes, cookies can be used to get information about what sites you visited.
b. No, cookies do not access the web page directly. Software which perform this type of activity are called 'Malware'
c. Usually its OK to keep cookies in your hard drive (even useful at times) as you might not want to tell a News website everytime about your location, rather you d like it to show the news relevant to you. So, keep that cookie on your hard drive and the website will do the rest.
d. Life of the cookie on hard drive depends on the cookie that is sent.
Hope this was helpful.

The main purpose of cookies is to identify users and possibly prepare customized Web pages for them. When you enter a Web site using cookies, you may be asked to fill out a form providing such information as your name and interests. This information is packaged into a cookie and sent to your Web browser which stores it for later use. The next time you go to the same Web site, your browser will send the cookie to the Web server. The server can use this information to present you with custom Web pages. So, for example, instead of seeing just a generic welcome page you might see a welcome page with your name on it.
