If you are in the US and you made a mistake -- especially one that is documented (you gave the wrong prescription, etc) -- then you can be sued by the family of your patient and worse, your medical license revoked
If you kill someone by accident, it depends on what you mean by accident. Did you accidentally cut a vein you shouldn't be cutting during operation? Did you accidentally prescribed a medication the patient is allergic to? Typically hospitals will investigate you and depending on the nature of the accident may suspend or revoke your medical license. There is also of course the threat of lawsuits.
In the US, spilling hot coffee on oneself due to one's stupidity is already reason to sue - how much more if you "accidentally" killed a person? This is a lawsuit happy country -- so no wonder doctor's liability insurance is going through the roof.
