You can't. The notes are writen on prescription paper. If you did it you would have to know how the doctor would write it. If it looks fishy they will call the doctor. You would be in double trouble then, with the school and the Doc.

don't forge, first of all--it will only get you in trouble in the long run. Talk face-to-face with your teacher-explain a problem-(even if you make one up!!!) Why, exactly, was the assignment late in the first place? If you really had a problem with it besides just laziness--you will find that most will understand.
