There's the classic run around the block naked. Also ask someone to call the operator and ask where babies come from. If boys are there you could always make them wear a bra and makeup.

Walk around for a specified period of time wearing a "cape" and declaring to passers-by, "I am Superman/woman!"
Do your best Billy Ray Cyrus impression for a whole minute.
In an audible, frustrated voice, argue with a mannequin in a department store.
When ordering a meal from a Chinese restaurant, ask for "Extra Cat."
Stick your finger up your nose and keep it there (for an hour).
Wish complete strangers a "happy belly button appreciation day."
(In an elevator...)Stomp invisible bugs when other people (strangers) are in the elevator with you.
Announce to five different people (during the course of a day) that you have new socks on (Optional: Don't wear any socks. =).
Wave frantically at different people (in a store, at a hotel, etc.) that you do not know, and act like you do know them.
Allow the darer to put makeup on the daree (They have 10 minutes to do as much damage as they can.), and then take a picture or go out in public for a specified period of time.
Propose to someone playing this round of "Truth or Dare" with you.
Pluck a sleeping person's eyebrows.
Paint a sleeping person's toenails.
Wave at passing cars on the interstate as if you know the people passing you.
In an elevator, rock back and forth on your feet humming "It's A Small World" with your fingers in your ears (and your eyes closed tight). Remember...you are trying to look SERIOUSLY disturbed.
