You've just about answered it yourself.
The following conditions favour elimination:
strong base, non-aqueous conditions (ie in ethanol), high temp, tertiary halogenoalkane.

The following conditions favour substitution:
milder base (or nucleophile) aqueous, moderate temp, primary halogenoalkane.
