Common reasons turtles die in captivity include small tanks, wrong diet, too hot or cold (the preferred range for captive turtles is 75-80F), and too much stress.

Head waving an mean anything from pain to neurological symptoms, to intestinal distress, etc.

Do you know the temperature of the water? Your turtle could have been overheating before it died. Res don't require their water to be heated. If someone told you this, they are wrong. Too much heat could have caused the turtle's brain to have gotten to hot. Especially a young slider. That could be one reason why the turtle was moving it's head around like you described. There, of course, could be other reasons, such as parasites.

Jacob has given you a good answer. Another possibility is that it could have had solmanilla, shell rot, or my guess would be that it overheated. It probably died of heat stroke or lack on nutrients. Just because it had UVB lighting doesn't mean that you fed it properly and gave it proper nutrients.
