Insufficient evidence to form a pattern? I've eliminated a clear pattern in another number base (in base 11, the first three numbers translate to 6 - 13 - 27, or 2x+1, but I'm not sure where things go from there). The problem is having only two transitions before the sequence leaves three blanks (transitions, not results).

However, this does imply that every transition must follow the same rule: with only two given transitions, there's not enough evidence to derive a differentiated rule.

Personally, I'd just solve it as a cubic equation through four known points: (0,6), (1,12), (2, 25), (5, 22). It fits the given data, end of problem.
