1- Do you know what does " lead implants" mean?
2- what does "Character" here mean?

1. I don't think it's lead (pronounced leed) as the answerer above does--I read it as lead as in the heavy metal (pronounced led). Just like lead weights weight down fishing line, his face seems to be drawn downward like there are small pieces of lead implanted in it.

2. I think the author is referring to the idea that age lines in one's face denote character--one's expressiveness has created them over time, therefore an aged face shows character.

3. The author then says that no, it's not character, it's erosion. Like water wears away soil in erosion on a seashore or riverbed, the metaphorical waves of disappointment and sadness have worn away at the man's face.

I think it's actually a bit of a mixed metaphor. I like the visual image of the lead implants, but it sets up confusion when the author starts talking about erosion. I think s/he probably should have gone for one image or the other, not both.

1. the skin of his face hangs down in pockets as if like tiny insulated wires (leads) that connect the device to the heart are steered through veins into the lower chambers (ventricles) to deliver the proper therapy.
2. people view that as sign of gentility and nobility
3. but i think it is not a sign of nobility but an indication of weariness from a lot of anxiety and depression.
