Both are substitutes for players already in the game. For example, a manager can send up a batter to bat for the player already in the lineup. The player is then out of the game, and the manager may replace him by the batter who was the "pinch" hitter or some other member of the team. The same scenario is true for a "pinch" runner, except that player is sent i to replace a player who is on base by way of a hit, error, walk, or hit by a pitch.

A pinch hitter is when the batter at the plate is substituted for a player on the bench
A pinch runner is when a baserunner is substituted for a man on the bench (usually because the baserunner out there is slow and the man coming into replace him is much faster)
In both situations, the player coming into the game cannot already be in the lineup, and once the play is subbed out, he cannot return to the game

pinch runner is a player who substitutes for a player who is currently on base, i.e a slower player is on base and you need to get into scoring position, you substitute a pinch runner who is faster and more capable of stealing bases or making it to base faster.
