I think HIV would bother me the most (for sad/depressing reasons, not because it's offensive). My parents almost gave me a name where the first two initials would have been V.D. but they changed their mind for the obvious reason. C.U.M would be pretty awful too.

EMT....I used to have these...I couldn't have a license plate with my initials cause I am not an EMT
I used to initial the girls school papers with ET....the teachers didn't believe it...thought my girls had initialed it.
