Huh? Only those currently IN med school are studying to be medical doctors. Those who have graduated med school ARE medical doctors. Those who have not yet entered med school are not yet studying to be medical doctors.

"What causes me not to go to med school?" Umm... you didn't get accepted?

"so everyone does go to med school?" No. Very few people go to med school.
