My cousin wants to change her first, middle and last name but she isn't sure about how to go about it or what it may cost, if anything. I couldn't answer the question for her, so I posted it here to get an answer.

Just go to any lawyer. Tell them you want a name change by deed poll. When you have the document from them, which takes about 15 mins and about 80 to 100 dlrs, you can change your name in the IC officially. I had mine done for 80 dlrs but that was a few years ago. Call them to find out about the rates first. Alot of conveyancing lawyers who do not do court cases charge a lower rate, especially the ones with business ( you dont need a good lawyer to do it anyway). Just a legal witness.

You can legally change your name at the courthouse. I think it only cost about $130 give or take. It may have changed price in the past few years, but it shouldn't be too much.

Have her look on the web for her local government services. My mother did this to change her name, and she just had to go to a local judge and tell them what she wanted to change it to and why. Then you get it changed with Social Security.

I think you have to go through the court system. Maybe even need a attorney or paralegal to start the process.
