Two possibilities. First, subject is called 'flatlander' by US rural mountain or hill person indicating he is city smart but country ignorant. Second, maybe speaker is referring to group who think world is flat. Term has absolutely nothing to do with sexual orientation.

it just means you live in either Alberta or Saskatchewan. not really an insult. they are one of very few places on earth that are so flat that you can watch your dog run away for 3 days.
