Does all women have a clit on the outside of the vagina or can someone have one on the inside?

Part of the clitoris is on the inside of the body, the rest is outside.

A really good book that shows some of the anatomy of the vulvar area is The V Book by Dr. Elizabeth Stewart. The pictures in there are real simple to understand.

If you are looking for something more technical check out the Atlas to Human Anatomy by Dr. Frank Netter. Your local library may have it, if not check to see if a Medical school library has accessibility available to the public so you can take a look at it in more detail.
