Do you have the descriptions for a 1-5 rating of work performance?

I am looking for the decriptors that read something like: Can perform independently without prompting,can complete task with assisstance, cannot complete task without assisstance, cannot complete task without extensive assisstance, cannot complete task.
This isn't the professional verbiage I am looking for, it is a generalization. Does your evaluation use a rating system like this that you can copy here? Thanks.

Looking for a 1-5 rating system specific for tasks instead of overall performance. Have seen it read more similiary related to the description in the question, but I just can't seem to get it exactly right.

1. Unsatisfactory.
2, Needs improvement
3. Satisfactory
4. above average.
5. Outstanding.

or
1. Fails to meet company requirements
2. Falls short of meeting company requirements.
3. Meets co requirements
4. Exceeds co requirements
5. Outstanding.

Now, to grade them.
Too many 1's and 2's, why do they keep you.
Too many 4's and 5'ss.,.the evaluation loses credibility.
Most people get mostly 3's, some 4's, and of course a 2. Your job is safe.
