The seal is broken between the panes and the gas has leaked out. I am sure if you went to a glass man they could replace the window. Even if it means the frame as well, it's worth it (picture window maybe?)
If the heat worked on the others, try a blow dryer(not toohot) to try to make the seal swell, but don't place any money on it!
If I might recommend, get a moisture meter and check the inside of your house. IF it sat there could be alot of it hidden in your walls and things, a dehumidifier will help depending on the percentage.

Good luck taking them apart as one guy suggested. You might as well separate the right side of your car from the left. What about manufacturers warranty on the window seal? You can replace the window pane without replacing the frame if you can find a local contractor that knows what he is doing.

Sorry, but they cannot be repaired. They are usually filled with argon gas at the factory. Air is replaced with the argon and then is sealed. Life of a window is about 5 to 6 years, before the seals dry out and the gas is replaced with air and moisture.

To the best of my knowledge, it can't be done. The window panes are sealed and when they get older, the seals crack allowing moisture and air in there. At this point you aren't getting the full benefit of thermal panes.

You can't., well you CAN; but the cost of re-sealing the panes is not economical in comparison of the cost of a new window.
it's just the added expense of new window installation.

You don't. there is a vacuum between the two layers of glass. You must have a leak in the seal.
