When you are as old as your grandmother, memories are you most cherished possession. You have passed the point of wanting much of anything from the store.
If you have a program like Photo Show on your computer, you could turn photographs into moving picture, complete with sound. You can have music in the background, or people talking. It is amazing what an individual can do on his computer nowadays.
Call your relatives. As them to send you photos in jpeg format, and have them identify the people in the photographs. Soon, you will have a very complete portfolio.
If you are less ambitious, you could just compile a bunch of photos, and put them on a DVD. The work involved would be the greatest present. The more effort you put into this project, the more she will enjoy it.

Here's something creative that will be appreciated and you will have fun doing:
Make up a crossword puzzle and have all the hints be personal inside moments and jokes that only you two would know about. Put it on a poster board and add some clipart and some pictures to it!
The crossword puzzle could be time consuming and a bit frustrating ... you could do the same idea with a WORDSEARCH or WORDFIND puzzle.
Put your computer skills to work! Use Excel, get clipart from Yahoo Images Search, copy them, and reformat them, use a gluestick to put on posterboard.
Use graph paper, and pictures from magazines if you are not as good on the computer, and it will give it a homemade feeling.

All of the answers are great.
But depending on your grandma's health, I think taking her out to do something different is nice.
How about a boatride where they serve a nice dinner or a play.
If you really want to spend, how about a short cruise.
If she has a hobby, take her shopping for some supplies for her hobby.
If she is interested in starting a new hobby, maybe a beginners's class in that.
I know that grandma's love their photos and memories, but I think they love quality time to make new memories even more.

I work at a carehome for elderly people. And the grandmothers love those calenders that you can make. Put pictures of the family or something special. And mark the days of peoples birthdays, anniverseries, weddings, etc. They just love it!!

I suppose it depends what you did to mark her 80th birthday but how about a framed family photo collage or a family photo album or a photo album of your grandmother at different times of her life? Or maybe just bake her a special, cake?

how about a paper or something with events listed on the day she was born. or get all the family together for a photograph, and put in a nice frame or photo album from when she was young to now.

you can go to walgreeans and get a oicture put onto a blanket, so get a family picture put onto a blanket.

Get her 81 roses or whatever her favourite flower is. I did this for my grandmother's birthday last year and she loved the idea!

If you can knit, make her a shawl. If you can cook make her favorite meal. If you can sew make her a hat. Just make something with your best handy work. I'm sure she'll appreciate any thing that her granddaughter gives her.
