Paraphimosis and phimosis are related conditions caused by inflammation and narrowing of the foreskin. The inflammation may be caused by infection and may be associated with poor personal hygiene. Occasionally inflammation develops after direct trauma to the area, which results in swelling.

If paraphimosis is left untreated, the blood flow to the glans is impaired. In extreme cases, this may result in loss of the penis tip.

I'd say a doctor's visit is a safe bet.

Paraphimosis is a medical condition where the foreskin becomes trapped behind the glans penis, and cannot be pulled back to its normal flaccid position covering the glans penis. If the condition persists for several hours or there is any sign of a lack of blood flow, paraphimosis should be treated as a medical emergency, as it can result in gangrene or other serious complications.

Paraphimosis can often be reduced by manipulation. This involves compression of the glans, then movement of the foreskin to its normal position, perhaps with the aid of a lubricant. If this fails, the foreskin may need to be cut (dorsal slit procedure) or removed by circumcision. An alternate method (the Dundee technique) entails placing multiple punctures in the swollen foreskin with a fine needle, and then expressing the edema fluid by manual pressure.

Prevention of recurrence is through education of the patient and his care givers on the need to pull back the foreskin over the glans after it has been retracted (for example, when cleaning the glans or passing a Foley catheter), or through elective circumcision or preputioplasty.

If your foreskin does get stuck behind the glans, there is a very easy and quick method to get it to return to it's normal position. Just firmly squeeze the glans between a finger and thumb until some of the blood gets forced out of it. It will then shrink in size sufficiently to allow the foreskin to roll back into place. Once you let go, the blood will return, and it'll grow back to it's normal size inside the foreskin. If you stretch the foreskin, it will become looser, and this won't be a problem for you. You can do it in the bath or shower - the hot water helps to soften the skin. When you’re erect, just ease the skin back as far as you can without hurting, as this can cause micro tears, which then heal with scars - not what you want. Hold it there for a few minutes, then let it relax. Repeat this as often as you can. When you’re flaccid, you can insert a clean finger under the foreskin, and gently stretch it, again, don't do it so much that it hurts. It will take several weeks, or even months, before you see any results, but keep it up daily and you'll get there. Pulling the foreskin back whenever you pee will help, as will masturbating. If this doesn't work, you can ask your doctor for some steroid cream, this will help to soften the skin, and make it stretch easier. Finally, ignore anyone who suggests getting a circumcision, they really don't know what they're talking about, a circumcision is almost never required for the problem you have.

Phimosis (a tight and not freely retractable foreskin is the main cause leading to paraphimosis (retracted foreskin being trapped behind the glans).

Circumcision is the long-term cure and also a preventative measure for paraphimosis.
