Haute couture is french for 'high dressmaking/sewing.' -- a custom-made item made of high-quality fabric sewn with care, attention to detail, and finish. Couture dresses are made by high-price point designers like Galliano and Gucci.

It's custom-fitted clothing (not mass produced). It's very expensive because of the fabrics used and the detail. Some people use it to describe ready to wear but it's not the same. Dior and Chanel have couture lines and ready-to-wear lines.
A couture dress is not mass produced, because of that the price is very high. Sometimes the dresses aren't that fancy (like a little sun dress), and sometimes they are (like some of the dresses on the award shows). It's mostly the material and detail (beads, print and all that other stuff) and it's not machine made.

It means a designer dress--one that was tailored specifically for that persons body. Couture is generally quite expensive but looks great and is unique.
"Couture" is short of "Haute Couture" which is French for "high dressmaking"--or something like that? I don't know french.

I think the term "couture" is generally used to refer to runway styles, but I suppose they could be worn out.

Yes, high fashion, high price, usually one of a kind, made by "the" designers with the best of material. They are the ones you see on the models on the runway and shown in the very exclusive shops. They are designed especially for actresses for the oscars, etc. These designs are copied and mass produced with cheaper material and sold for less off the rack so smucks like me can afford them.
