I have heard of people boasting about someone they know graduation with a 4.2. How is this possible?

Some Universities will allow you to graduate with above a 4.0. Many colleges base GPA off of the whole letter grade. Others will account for the +/- that you earn. Under such a system, the best grade you can get varies between a 4.5 and 4.66 for an A+. Such a tactic may or may not motivate students to put forth the extra effort for half a letter grade.

It isn't. At least not at a reputable college. Some, less reputable, schools will give you "an extra boost" in your GPA for an A+ (like a 4.3/4, but most think that A+s are just silly.

if you are in honors classes your grades could be weighted which means that an A would be worth more than 4 points giving you a higher GPA

A 4.0 is 100, how can you get higher than that? I've never heard of it.

Not at most colleges. Usually honors classes in college offer no GPA boost, they are just harder classes.
