It means that one parent has custody of children/offspring, without the input or requirement of the other having the option to have the children live with them. The one that is responsible for the main decision making for the children is the custodial parent. That one has the children for the majority of the time; the children live with that parent, and usually, the other parent (or grandparents) only have visitation rights to see the children.

MJ is right and you will have to add that in that case the one having custody is the one that decides about the education, school and everything related with the child, without having to consult the other.
