I dont want a date or anything like that. I nearly lost my wife and my two young children. I was thinking of just the word "CHOOSE" in bold lettering to help remind me of what I almost lost. I am open to any suggestions, or opinions.

How about the mineral symbol for alcohol superimposed onto the international symbol for stop. You know the circle with the slash through it.

One or more of these things will make her think you are an awesome boyfriend: A letter or card that lists things you love about her (even things like her hair or the way she pronounces certain words, or the way she laughs, etc.). A letter or card that describes your first impression of her when you met. A picnic in the park. Tell her you'll do anything she wants for the entire day (even if it's shopping, watching a chick flick, or calling her Honey Bunny all day) Send her an email (on the morning of your anniversary) with an attached mp3 file of the song that reminds you of her. Draw her a bath (clean the tub, run a warm bath with extra bubbles, lay out fresh towels, light candles around the tub, put on some music that is not turned up too loud, set out a glass of champagne/beer/mixed drink/sparkling grape juice) Buy her some flowers (they have them at Wal-Mart for $5 and up). Then sit back and enjoy one very happy girlfriend!

perhaps you or something symbolizing you busting from a bottle. I like the CHOOSE, Sometime you will be depressed or facing family problems as we all do, and be tempted to choose the wrong answer as a knee jerk reaction. Victory, survivor, freedom, overcomer etc. all apply to you. Whatever you select, you have reason to be proud, congratulations.

Get something that only you will know what it means, like an animal that reminds you of strength, of something that reminds you of your family.
It is great that you are sober for a year. Good for you!

"Choose" is kind of boring.
I would use symbolism.
Libra scale could represent deciding between family and drinking. You could even have the right side of the scale low meaning you have chosen to do the "right" thing.
