why do you have to turn off the filter when you are treating the fish for ick?

NO no, you absolutely must run the filter - you want as much water movement and aeration in the tank as possible when treating a fish.

It's just the carbon that you want to remove, because activated carbon will remove medication from the water.

You don't have to actually turn off the filter - just take out the carbon insert. The carbon will filter through the medication so that your fish aren't getting all that they need.
