APPLYING FOR YOUR FIRST PASSPORT
To get your first passport, you need to go (yes, I mean in person) to a passport agency. Bring with you :
* Two photographs of yourself;
* Proof of U.S. citizenship (ex: your birth certificate);
* A valid form of photo identification such as a driver's license;
* A passport application form.
Passport agencies are usually a post office, government office or public library. You may not apply for your first passport online. To find an agency near you, visit: http://iafdb.travel.state.gov/.
Your first passport will get you through the next 10 years and will cost $97 if you're age 16 or older. Child passports last for five years and cost $82.

You left out some information, but I'll answer to what I know.
If you are flying domestic, meaning you are traveling within the US, you don't need any documents.
If the child is not your child but a relative, and you are traveling with the US you need a signed letter from the parent stating that you have permission to travel with their child.
If you are traveling international, you may need a birth certificate, as customs is more difficult since 9/11

This depends on where are you now. If outside USA you need to contact, USA consulate or embassy and get a passport. If you have a USA passport then easiest thing may be an endorsement on your passport. If you have any other passport and that has the endorsement for your child then check with both countries embassy or offices.
At one time if the child was 2 years old then USA was requiring an individual passport, but rules change from time to time.
If both of you are in USA and are not going outside USA, then you do not need any passport but to fly you need a valid identification only. So if you do not have a driver's licence then you will need some thing else check with airline. They may need proof that it is your child that you have with you, so check for every thing.
In these times of trouble it is good to have valid identification on you at all times. So get going.

If you are traveling within the United States, all you need is a birth certificate for the child, and a photo ID for one of the parents on the birth certificate (i.e. your passport). Also, you can go to your state's department of motor vehicles and get a state-issued identification card for your child which will help you get through more difficult airport security.

you are telling two year old US citizen.Then to travel in Us for her no passport is necessary.
but I think that you are proposing to travel with her outside Us, perhaps to India.In that case YOU NEED TO HAVE A US PASSPORT FOR THE 2 YEAR OLD BABY.IT IS A MUST

As long as you stay within the USA, you will not need a passport - nor will your child. Passports are only required to cross the border between countries, not within a country (or at least within the USA).

YOU DO NOT NEED TO HAVE A PASSPORT WHILE TRAVELING IN THE US. A BRITH CERTIFICATE WILL WORK FINE. HOWEVER, IF LEAVING THE COUNTRY A PASSPORT IS REQUIRED FOR MOST LOCATIONS.
