Go to www.cdc.gov - center for disease control. They have recommendations for which vaccines you should get before leaving the US. It depends on where you are going.
My understanding is that you don't NEED them, but to ensure your health upon return, you SHOULD get them.

None. You are not required to have shots to leave the country.
Certain places require certain shots before entering, specifically Yellow Fever vaccinations in many Tropical Jungle areas. You should go to the cdc website to see if there are any shots or other preventative medication for the areas you will be travelling to.

It depends on where you're going. The only vaccination that is generally required is Yellow Fever and then only in specific countries and circumstances.

Depends on where you're going. You'll have to consult the rules for entry into the country where you're travelling. When I went to Europe, I didn't need to do anything, or even get a visa.
