I guess you could say that superconductivity and superfluidity involve phase transitions....

If this is a homework question, I feel bad for you and your classmates. Tell your teacher he/she sucks for me.

Dark matter would be the fifth substance observed based on measurements and recession velocities of Universal galactic expansions.
In reality it should have been called the first substance which was actually what Aristoltle & Democritus described as existing in indivisible corpuscular form.Which later was called the Aether ;but was never correctly conceived or understood by Michelson & Morley.
Einstein ,However did attribute properties to that substance and indicated a Universal type of force which existed to form both the electrical and magnetic phenomena.
Neverthe less he was not really able to describe any of the mechanisms of gravity or electromagnetism into one common cause and effect system.

Liquid, gas, solid and plasma are not measurements of matter, they are forms/phases of matter and the newest form of the matter created by physicists on March 20th, 2002 is Bose-Einstein condensates. Read this article: " A new form of matter." http://science.nasa.gov/headlines/y2002/20mar_newmatter.htm
