I would say that the top 4 places are
MacDonalds
Burger King
Taco Bell
Dunkin Donuts
You don't have to focus on these 4 but they give you enough info about how bad the nutrition is.
Information you should include is that all these companies use the worst oils possible, all saturated fats, they do not use trans fats, which are the fats that are good for you (Canola oil, olive oil, peanut oil), they deep fry their foods and don't care how bad it is for the customers, they use inferior meats where applicable. They use only white flour which has absolutely no nutritional value. There are other flours out there that have nutritional value, sourdough, rye, spelt, and several others, but they cost more, even though they taste the same. You probably have never heard of spelt, but it tastes basically the same as white bread and it is extremely good for you, it doesn't add wasted calories and metabolizes in your system in a good way instead of just adding useless weight. Most of these companies add useless salt and sugar to enhance the flavor, these would not be needed if they used better products. It has always been known that if you use low grade meat all you have to do is add a lot of salt and it will taste much better, even fancy restaurants do this.
I hope I have been of some help to you. Just remember, every time you go to a fast food restaurant and order one of their regular meals, that is something like a whopper, fries and a coke (with sugar) you are getting about 1,600 calories, which is almost all the calories you should have for one day.

Question 1.
1. McDonald's
2. Burger King
3. Wendy's
4. Taco Bell
Question 2
No you should include more then one but you don't have to give to much info but just touch up on others.
Question 3
You should include the average money spent at each fast food chain....
also what might be fun is to include how much is spent on average at different fast food topics like .... pizzas , burgers, Chinese food etc etc etc.... so in all you would find out how much people tend to eat most of good luck

If I were you, I'd go to the top 4 fast food places the kids go at your school. Get some easy local feedback and real quotes. That will really liven up a otherwise mundane paper.
The key in a school paper really isn't the hard facts, it's how entertaining and easy to read you make it for your teacher.
