I'm only familiar with "Off the Hook", which would mean out of danger normally associated when blame for an event is being allocated. May correspond to a fish escaping becoming a meal.

"Out of the Hook", I would associate with navigating a river, where a hook has rapid current and potential for sand bars especially on the inner, 'preferred' side. So there would be great relief when the pilot was, 'out of the hook'.

This is just my guess, but I bet Mark Twain would know if I'm right.

I think you mean 'off the hook'. It means being relieved of some obligation, ie. you no longer have to do what had been expected of you.
