If you have a host (which you will need) then you need to upload all folders and files that comprise your website to your root (public_html) folder of the host. You would use most commonly a FTP program (file transfer protocol) or an http upload provided by the host service. Using an Ftp prog, you would login to your host thru your IP address/Host address/Your URL and the root should be pretty apparent then. Usually the host will provide very detailed instructions to the exact address you need for your root folder. Most free hosts typically have a form you browse on your PC for your files and upload via their interface.
At the least, the simplest webpage has to have an index.htm(l) page. there are other ways around this but that is another convo.

publishing a website can be confusing at first but once you understand how its done you'll get the hang of it. Basically a webpage needs to be hosted on a server somewhere, meaning a computer that is not your own holds all the files that make up your page and then sends them to whoever wants to check it out. So you'll need to find a place that is willing to store your new website. There are many out there that are free but usually force you to have advertisements on your website (if you want search free hosting on yahoo) These will usually give you a page like www.free-company.com/yourname I believe yahoo owns a free hosting place called GeoCities. Then there are other company's that you pay a monthly fee to. If you pay you'll generally get more storage space, a www.whatever-you-want.com address and will not be forced to have any advertisements.
To actually publish your website it generally works like this:
Free places usually do it all through a control panel on their website that is availible once you login. You simply click an upload button there, browse to your website files, and upload them. Then it will be published
Places you pay will usually almost always use FTP (file transfer protocol) For this you need a FTP program like the one built into firefox or a entirely seperate program like BulletProof FTP or CuteFTP. Here you would enter the servers name, your login name and your password to connect. Once connected you locate the website files from your hard drive and then transfer them through the FTP into your webspace.
Also if your using a program to create your website (like DreamWeaver, FrontPage, etc..) there are often built in publishing features. These basically use the FTP described above only the program simplifies it and makes it more automatic.

There are many programs that you can put files to a server with you could get a better answer if you would say what type of software you are using and as much info about it you can I'm a web designer and i don't even know what you are talking about. But basically all websites have a log in and a password once you get that all in you are in the server. But there is a hierarchy of folders and files that you need to understand. Your question is so vague it hard to really understand what you are having a problem with I could explain everything but it would be like writing a book. So please add some details

That would be the place that is hosting your page. You may be using the space your ISP provides, check with them to know the server you should send it to. If you are using one of the many free webhosting sites available on the internet, check through their FAQ's. They will usually publish specific instructions for uploading your webpages. If you're using a home server such as apache, just put it in the directory for the webpages (htdocs on apache2)
