Yes. When a woman goes into labor, the sac of fluid (called the amniotic sac) surrounding the fetus ruptures in preparation for the baby to be born. The amniotic sac forms inside the uterus and surrounds the fetus throughout the pregnancy to protect it.

Oh yes..sometimes on its own, and sometimes with the Dr's help.
What happens is that the sack surrounding the baby bursts under the pressure of labor, and the remainding amniotic fluid is pushed out. It isn't necessarily a HUGE amount of fluid, sometimes just a few ounces, sometimes a cup or two. Hope this helps.

Yes, a pregnant woman's water really does break. It may not always happen on its own. When this occurs then the dr needs to intervene.
