It's the mayonnaise. Mayo is made primarily of water, oil of some kind, and eggs (and other emulsifiers, usually) and spices. The ingredients are madly scrambled to make a thick emulsion, but in the end result it's mostly oil. So, it's high in fats and cholesterol. There is debate among some whether or not the tuna is oil- or water- packed, but if indeed it's oil-packed, that only makes it worse.
At home, if you use lite mayo and water packed tuna, you're good to go.

Probably you get the tuna in spring water. They'd use the tuna in oil and the regular mayonaise, not the lite mayonaise. Also if they put the cheese on it that also adds to it. White bread also has more claories than whole wheat if that's what you get.

It may be that the Tuna they use is packed in oil and not Spring Water. Additionally it could be because of the Mayonnaise that they use.

Its their meat products. Most commercially available process meats are heavy in salt/sodium. Part of the reason why, salt keeps the meat from spoiling as fast. Usually customers order a meats sub and add salt+pepper to the meal, increasing the sodium intake. Its not only Subway but most fast food contain high sodium.
