Depends on your level - Murphy (Elementary Grammar in Use) is a good start and there are Intermediate and Advanced versions.

Murphy has a one page explanation followed by exercises (answers at the back) - it's the book most new English grammar teachers carry as the explanations are very clear.

I use Easy Grammar by Wanda C Philips. It is an excellent workbook style tool. My children were diagramming sentences in no time flat. I bought mine in a homeschooling store in a nearby city, it is also available in homeschooling catalogues. The teachers edition have the answers in it as well as the workbook. It is self explanatory and easy to use.

holt handbook is a great guide. try amazon or borders. or a used school book store in a local college.
